Acceptance as a patient with Myleaf is subject to clinical suitability, regulatory requirements, and practitioner availability. We do not guarantee that you will be accepted as a patient. Your eligibility will be determined through a clinical consultation with one of our authorised healthcare practitioners.

Voluntary Participation:
This offer is entirely voluntary. You may choose not to proceed at any time before your consultation however the discharge letter request will be sent on acceptance of offer. If you do not wish for us to contact your current provider, you should not proceed with the discount offer.
